Nuala arrives in New York City and takes refuge at the BPRD headquarters. Hellboy and his team are sent to investigate the auction massacre and encounter Nuada and Wink. During the fight, Hellboy injures Nuada and receives a wound that does not heal. He also discovers that Nuala and Nuada are connected by a psychic bond, which causes them to share wounds and thoughts. Nuala and Abe Sapien, an aquatic humanoid agent, develop an attraction for each other. Meanwhile, Hellboy’s relationship with Liz Sherman, a pyrokinetic agent, suffers due to his reckless behavior.
Nuada tracks his sister to the BPRD and battles Hellboy for the final piece of the crown. Nuada critically wounds Hellboy with his spear and escapes with Nuala, promising to release her if she gives him the crown piece. The spear contains a rare poison that threatens to kill Hellboy, but Liz pleads with an angelic being known as the Angel of Death to save him. The Angel warns that Hellboy will doom humanity if he lives, but Liz accepts the risk and receives a vision of Hellboy’s true destiny: he is Anung Un Rama, the beast of the Apocalypse.
With the help of a map given by a friendly troll market elder named Mr. Wink’s father ( Roy Dotrice ), Hellboy and his team find their way to the hidden realm of Bethmoora where Nuada awaits them. They are joined by Tom Manning ( Jeffrey Tambor ), head of the BPRD, who wants to take over the operation. They confront Nuada at the chamber of the Golden Army, where he challenges Hellboy to a duel for the right to command the army. Hellboy accepts and defeats Nuada, but spares his life. Nuada tries to stab him from behind, but Nuala intervenes and kills herself, along with her brother.
Hellboy decides to quit the BPRD after Manning berates him for his actions. He is joined by Liz, Abe and Johann Krauss ( John Alexander ), an ectoplasmic medium who inhabits a containment suit. Liz reveals that she is pregnant with Hellboy’s twins and they embrace.
